Getting Ready for a Plan Review

A plan review can feel a bit like a school report — a moment where everything you've done gets looked at. But it's really something kinder than that. A review is simply a chance to talk about how the last plan went, what's changed, and what you want for the year ahead. It's an opportunity, not a test.

With a little preparation, you can walk in feeling calm and clear about what you need. Here's how to get ready, for people and families across Mortlake, St Marys and the wider Western Sydney area.

What a plan review is for

A plan review looks at two things: how your current plan has worked, and what you need going forward. The conversation usually covers:

  • Progress toward your goals
  • What support helped, and what didn't
  • Any changes in your life, health or needs
  • What you'd like your next plan to include

Going in with thoughts on each of these makes the whole meeting flow more smoothly.

Reflect on the past plan

Before the review, take some quiet time to look back. Ask yourself:

  • Did I make progress on my goals?
  • What support made the biggest difference?
  • What didn't work as well as I'd hoped?
  • Did my funding last, run short, or go unused?

There are no wrong answers here. Even "this didn't work" is valuable — it helps shape a better plan next time.

Gather your evidence

  • Updated reports from your GP, specialists or allied health team
  • Notes on how your needs have changed
  • Records of what support you've used
  • Anything that shows progress, or new challenges

If your situation has changed since your last plan, recent reports that reflect that are especially worth having.

Think about your goals for next time

Your goals drive your plan, so give them real thought. Consider:

  • What do you want to keep working toward?
  • Are there new things you'd like to aim for — more independence, community, work, a new skill?
  • What support would help you get there?

Make some notes to bring

On the day, nerves can make things slip your mind. A simple written list helps you remember:

  • Your key points about the past plan
  • The supports you want to continue
  • Anything you want to change or add
  • Questions you'd like answered

Even a few bullet points on your phone can keep you on track.

Bring someone you trust

You don't have to do a review alone. A family member, friend, support coordinator or advocate can:

  • Help you remember the things that matter
  • Speak up if you find it hard
  • Take notes so you can focus on the conversation

Two sets of ears really are better than one, especially on a day that might feel a bit nerve-wracking.

Speak openly and honestly

The most helpful thing you can do in a review is be honest — about what's working, what's hard, and what you need. It can be tempting to put on a brave face, but the review works best when it reflects your real, everyday life, not just your good days.

If you need more support, say so. If something isn't working, name it. This is your chance to shape the year ahead.

After the review

Once your new plan arrives, read it slowly and make sure it matches what you discussed. If something doesn't look right, you have the right to ask questions or request a review or change to your plan. Don't just accept a plan you don't understand.
